Job Description:
A company is looking for a Clinical Pharmacist to support market
operations through patient-centered clinical consultations and
provider engagement.
Key Responsibilities:
Conduct telephonic patient outreach and provider-facing activities
to improve STARs measures
Analyze STARs and HEDIS reporting to provide insights and best
practices to provider groups
Perform annual medication reviews and develop plans to enhance
medication adherence
Required Qualifications:
Doctor of Pharmacy degree or equivalent experience
Active, unrestricted pharmacy license in the state of primary
residence
5+ years of clinical pharmacy practice experience
2+ years of experience with Medicare STARs and HEDIS measures
2+ years of experience in provider-facing interactions and data
analysis
