Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Title: Physical Therapist – Skilled Nursing Facility (SNF) Location: Lancaster, CA Duration: 13 weeks Shift: Day 5x8-Hour (08:00 – 16:30) Job Responsibilities and Required/Preferred Skills & Requirements: /t - /t Provide physical therapy services to patients in a Skilled Nursing Facility setting /t /t - /t Evaluate, plan, and implement individualized treatment programs to improve mobility, strength, balance, and functional independence /t /t - /t Treat patients with orthopedic, neurologic, and post-operative conditions including joint replacements, spinal conditions, stroke, Parkinson's disease, traumatic brain injury, and spinal cord injuries /t /t - /t Perform assessments related to body mechanics and functional movement /t /t - /t Develop and educate patients and families on home exercise programs /t /t - /t Document evaluations, progress notes, and treatments using facility-approved charting systems /t /t - /t Collaborate with interdisciplinary teams to support patient-centered rehabilitation goals /t /t - /t May float to other care settings as needed (acute care, outpatient, rehab hospital) /t Required Qualifications: /t - /t Active Physical Therapist license /t /t - /t BLS certification /t /t - /t Minimum 1 year of experience preferred (new graduates will be considered) /t /t - /t Experience or interest in Skilled Nursing Facility (SNF) setting /t /t - /t Ability to manage a diverse rehabilitation caseload and adapt treatment plans accordingly /t Preferred Qualifications: /t - /t Prior SNF experience /t /t - /t Experience with orthopedic and neurologic rehabilitation populations /t /t - /t Strong patient and family education skills /t HonorVet Technologies Job ID 31769036.
