CareConnectMD DCE is a specialized High Needs Direct Contracting Entity (DCE) geared towards Medicare beneficiaries who reside in a nursing home. Developed by geriatric professionals, the program provides a care model that is designed to meet the unique health care needs of nursing home residents. Under this value-based care model, CareConnectMD DCE will deliver care coordination services in close collaboration with primary care physicians, specialists, and advanced practice professionals in California, Georgia, Ohio, Indiana, Texas, and others on the horizon.

The Community Outreach Specialist promotes the DCE program and services in accordance with established policies and procedures. The position is responsible for supporting business development efforts by assisting in the development and growth of the provider network and the patient census. The Community Outreach Specialist builds relationships with nursing facilities and other providers in order to support the continued growth of the DCE. As a key member of the business development team the specialist provides administrative support and assists in planning and implementing growth campaigns. Key Duties and Responsibilities
Supports Voluntary Alignment efforts
Outreaches to provider offices and enhances positive working relationship with support staff
Builds positive, solid working relationships with members of the medical community and partners
Works with nursing home partners to increase provider and patient participation
Provides administrative support to include campaign planning and implementation, mailings, meetings and follow up activities
Maintains metrics on campaigns, outreach efforts and patient growth
Makes regular, planned calls to build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in nursing facilities
Prepares, sends, and manages general correspondence with internal and external stakeholders
Assists team with communications and coordination of events
Plans meetings and manages vendors and event quality
Maintains confidential files Education and Experience
Associates or bachelor's degree in related field preferred or 3 years of experience in marketing outreach or administrative support
Excellent organizational skills
Experience in collaborating closely with clinical staff and physicians
Excellent organization skills
Highly collaborative; great communicator
Skilled in Microsoft 365 (Word, Excel, Power Point, Teams, Outlook)
Proven ability to generate and maintain relationships with new and existing accounts including physicians, healthcare professionals, and community agencies.
Bilingual - Spanish
